Updated Fields Patient Limit and Insurance Limit

We introduced a new enhancement in POS and contract types that updates how patient and insurance limits are managed, making insurance handling clearer and easier for clients.

⚙️ Functionality
we renamed the field “Insurance Max Limit” to “Patient Max Limit” for clarity.
we also added a new “Insurance Max Limit” with updated logic:

- if Patient Max Limit is used, it overrides contract type percentages, and the remaining order amount goes to the insurance share.

- if Insurance Max Limit is used, contract type percentages apply only to the specified amount, and the rest is added to the patient share.

- both fields cannot be used together in a single order; using one disables the other
